Provider Enrollment Form

Please submit one form per individual provider in your practice. This project seeks to plan, implement, evaluate, and recommend improvements to the behavioral health integration system for children in primary care settings in Leon and surrounding counties in Florida. The behavioral health integration model will comprehensively address components of primary care, behavioral health, specialty care, and the social support needs of children and youth ages 3-21 with behavioral health issues which cannot be solely managed at the primary care level in a manner that is continuous, family- centered, and collaborative. The project will partner with pediatric care practices to conduct needs assessments, trainings for providers at practice sites, and develop a customized, evidence-based behavioral health integration model for implementation at each of the providers and/or practice sites.

What will you be expected to do?

Enrolled providers are invited to collaborate with project staff from the Florida State University (FSU) -Whole Child Leon (WCL) Behavioral Health Hub: 
    • To work with project staff to develop a customized Provider Transformation Plan for implementing the behavioral health integration model;
    • Collaborative activities include attending quarterly evidence-based skill building trainings, receiving ongoing email communications with FSU and WCL, and completing brief pre and post training assessments;
    • To provide feedback and insight on community pediatric behavioral health resources and information to inform broader dissemination of the collaborative model across the state.

What are the benefits for the provider?

Enrolled pediatric providers will receive training and consultation with the goal of increasing the number of children and adolescents identified as receiving needed mental health services.
